Veterinary Reference

To help ensure the safety and well-being of our adoptable pets, we require a current veterinary reference for all adopters. This helps us confirm that the pets in your care receive proper, timely veterinary care and that your household information is accurate.

Important: You are responsible for contacting your veterinarian in advance to provide consent for them to speak with Forest City Rats (Second Chance Program) about your account. We will not chase you or your vet for this.

If you do not have a current veterinary reference, we will ask for some alternate information. Note: Applications without a veterinary reference may take longer to process, and approval will depend on the strength of your references and experience.
