By signing this form, I confirm that:

• I am the legal owner of the pet(s) I am surrendering, or I have full permission from the owner to act on their behalf.

• I understand that submitting this form is a request for surrender and does not guarantee intake. Acceptance is dependent on Forest City Rats’ current space, resources, and capacity.

• I understand that the information provided in this form will be used to assess suitability for intake and to support the pet(s)’ care, fostering, and rehoming process.

• If accepted, I agree to provide the pet(s)’ current enclosure (cage) and any available supplies to help minimize stress and support their transition.

• I understand that I am responsible for arranging and completing the transportation of the pet(s) and their enclosure to Forest City Rats. No pickup or transport services are offered.

• I agree to disclose any known health concerns, behavioural history, or bite incidents to the best of my knowledge.

• I acknowledge that once the pet(s) are surrendered, they become the full responsibility of Forest City Rats. Ownership is permanently transferred, and I will no longer have any rights, claims, or decision-making authority regarding their care, placement, or future.

• I understand that Forest City Rats reserves the right to make all decisions regarding veterinary care, fostering, and final placement in the best interest of the pet(s).

• I affirm that all information provided in this form is accurate and truthful to the best of my knowledge.
